Care plans are live documents, they change and evolve constantly and so Cortoncare makes creating, changing, tracking and communicating care plans the easiest it’s ever been!
To get you started, Cortoncare will automatically create a “start point” for a care plan from the resident’s pre-assessment whilst also creating a reminder to alert the relevant care manager that the care plan needs a review.
A full audit trail allows sight of when care plan revisions were made, what has changed, and which member of staff made the adjustment. Individual entries within the care plan can be highlighted and automatically included in a “This Is Me” view or report.
